1. Services
Ad Hoc Occupational Health Limited will provide all clients registered on the “Resolve Attendance Management Portal” with:
1.1 Pre-placement health assessment questionnaires on demand and as required.
This involves the issue of a “certificate of fitness” recommending any appropriate action within 1 working day of a candidate submitting a completed online questionnaire. This is triaged by an OH clinician and referred to an OH Physician (OHP) where appropriate. Liaison with the employee’s GP or consultant specialist where further medical reports are required, and provision of a report on the employee where applicable (at an additional cost to be confirmed beforehand), may be warranted.
The questionnaires will be charged at £13 each (this fee includes telephonic follow-up with the candidate if required). An additional management fee would be payable should further medical evidence (FME) be required, plus the cost of obtaining FME as appropriate (please see section 5 of the service level agreement).
1.2 Attendance management referral service on demand and as required.
This involves a telephone consultation with an employee following an attendance management referral, and a management report recommending appropriate action in response to the questions asked (unless further medical information is required), within 1 working day of the assessment being completed. If the employee exercises their right to see the report before it is released to their employer, the referring party will be notified within this timeframe.
Management of referred cases via telephone contact will be charged at £125 for the assessment and report.
Management of referred cases in person and on-site will be charged at £250 (exclusive of mileage at £0.45 per mile) for the first assessment and report.
Please note that if more than 5 additional questions are asked by the referring party, a double-booking fee will be charged to allow additional time for the clinician to answer the questions.
The above does not include the cost of further medical evidence (“FME”) for which an additional charge will be made in accordance with the provisions set out in section 5 of the service level agreement.
For any appointments cancelled within 2 working days of the consultation, and missed appointments, please see section 6 of the service level agreement.
Did not attend (“DNA”) fees will also be charged to the client in full.
Other services provided by Ad Hoc Occupational Health Limited include:
1.3 Annual food handling, hand-arm vibration syndrome (HAVS), and night worker assessments. This involves the issue of a “certificate of fitness” or recommendation of appropriate action following completion of a health questionnaire (online). This service will be charged at £13 each.
An additional management fee will be payable in the event that further medical evidence (FME) is required, plus the costs of obtaining FME as appropriate (please see section 5 of the service level agreement).
1.4 On-site pre-employment, and annual health surveillance medicals. For employees regularly exposed to those risks requiring them to undergo routine statutory health surveillance, under Health and Safety Executive (HSE) regulations the following tests are available: blood pressure, keystone vision, audiometric hearing, tiers 1 (baseline) and 2 (annual) hand-arm vibration syndrome, skin checks, and lung function.
On-site Pre-employment and Annual Health Surveillance Medicals will be charged at £500 per day, £750 per night shift and long day shifts (exclusive of mileage). Overnight accommodation costs may also apply based on the location of the premises to be attended.
Referrals from the health surveillance programme that are conducted on-site, will be charged as a face to face management referral as detailed in section 1.2 of the service level agreement.
1.5 Safety-critical worker assessments. This includes drivers of commercial vehicles (Group 1 license, e.g. forklift truck or crane operators, and Group 2 license, e.g. lorry or bus drivers). We recommend an on-site medical to ensure eyesight standards meet the legal requirements mandated by the DVLA.
Safety-critical Worker Assessments will be charged at £100 during business hours (9am to 5pm Monday to Friday) , £150 outside of core business hours and £200 on Sundays (payable upon booking and subject to clinician availability).

2. Additional Services
Any of the following services will be provided by a third-party supplier and will be charged at cost unless agreed otherwise prior to the service commencing.
2.1 The Rightsteps Wellbeing Programme is delivered in the form of telephonic and online counselling, with a management report to be released on completion.
3. Payment for Services
For any service provided by Ad Hoc Occupational Health Limited, the client agrees to pay the service provider as and when they use such services or before the release of the requested report/certificate.
The service provider will invoice the client before any services are commenced, and the client shall pay all such invoices within 30 days of the due date given. Please note, where invoices are not settled by the due date, a statutory interest charge of 8% plus the Bank of England Base Rate will apply.
4. Additional Fees
If a call to discuss a management referral or report is required, please note that the first 30 minutes of the call will be charged at £50 and then a charge of £100 will apply if the call lasts up to an hour long. Alternatively you can discuss the referral or report via the “Resolve Attendance Management” portal at no additional charge.
5. Further Medical Evidence (FME)
The cost of any further medical evidence will be charged as follows:
5.1 Management of referred cases via OHP of employees across the UK will be at the cost indicated by the service provider.
5.2 Reports from an employee’s General Practitioner/treating specialist will be charged at cost plus a £100 administration fee to cover the cost of our report to the clinician and of our subsequent report to management following the receipt of said report.
5.3 Workstation assessments (“WSA”) or workplace assessments (“WPA”) will be at a cost indicated by the service provider.
If the Service Provider considers it necessary to arrange a medical assessment with an OHP, the client will be given prior notice that an additional charge will be payable, and the client may instruct the service provider not to proceed.
6. Cancellation/changes to bookings
Full charges for management referrals and safety critical medicals will apply to appointments not attended and for cancellations or changes to appointments with less than 48 hours notice (excluding weekends and Bank holidays).
For heath surveillance, where a client cancels or amends the booking that has been agreed with less than one weeks notice, full charges that have been agreed will apply.
7. Medical Records
The following charges relate to medical records:
Ad Hoc Occupational Health Limited is obliged by law to supply, within 30 days and without payment for the first copy, any data pertinent to an individual requesting its release in writing under the General Data Protection Regulation (GDPR).
We reserve the right to levy a charge of £10 for releasing further copies of this information electronically to cover our administrative costs, and up to a maximum of £50 if paper-based health records are requested (depending on the number of pages provided).
8. Cancellation of account
In addition to the above, should the client wish to cancel their account, they are required to notify the service provider to enable transfer of records if required and collation of their medical records for download (at a charge of £25 to cover the administrative costs), before the account can be confirmed as closed.
